IMPORTANT: All students must have a new physical copy of the medical release form filled out for 2025-2026 as well as the Falls Creek Medical Release Form. The Emmaus Medical Release Form will be coming soon. Both forms are paper only. See the links for the form below the registration link. Print the form and turn it in to the Student Ministry office.
